Accurate and complete patient records are essential to good chiropractic care and your safety. Your own unique, permanent patient record is treated with professional confidence. Its purpose is to provide a valuable reference for any chiropractic doctor and referral physicians you may consult with. To assure proper personal identification, we ask that you pre-register with us when you make your appointment. When you arrive for your initial consultation, we will have your initial chart prepared for review by your assigned doctor.

We recommend that you pre-register you and/or your family before your first consultation visit. This can be done by phone, mail or you can come to our clinic approximately 15 minutes before your first appointment. You may also pre-register online by clicking the appointment link above.

In order to prevent confusion and assure accuracy, we will need your full name, date of birth, provincial Personal Health Number (PHN from MSP), address, phone number and other pertinent information.

If you were previously under another doctor's care outside the Bayside Chiropractic Group, please send a written request to that doctor to transfer your records to us. You may obtain a medical records and x-ray release form to assist you in your request by clicking the appointment link above.
