We aspire to treat all of our patients with the same consideration that we desire for ourselves and our families. All of us at Bayside Chiropractic consider it a privilege to work with you and we are delighted to have you as a patient!

1. Paperwork
When you come for your first visit, plan to arrive 15 minutes early to complete the patient registration paperwork. You will be required to fill in some forms with your personal contact information and medical history. Please be prepared to show your Personal Health Care card if you are registered with the Medical Services Plan (MSP) or other health care services plan.

You can choose from two ways to register as a first time patient:

1. In person: Complete the required forms when you come in for your initial consultation.

2. At home: Download and print the required forms here and bring them in with you.
You will need Adobe Reader to use the forms.
If you do not have it, get it here:

2. Consultation
The comprehensive consultation will usually take 45 minutes to 1 hour. This will initially include an in-depth history of your chief complaint and any other additional complaints. After that, a physical, orthopedic, and neurological examination will follow. The doctor will also take your blood pressure and check your ranges of motion to help determine if your problem is indeed a chiropractic case.

3. X-Rays
The doctor might require x-rays to be taken after your initial consultation. This is usually recommended if there are any signs of pathology, malformations or fractures in your spine. If there has been a history of trauma, x-rays will also be required. Other general condition of the joints in the spine such as scoliosis, osteoarthritis, and conditions with neurological symptoms will also warrant a set of films. Your doctor will discuss this with you first.
